We are so glad to have you joining our team as a Mentor. Before you can begin supporting our participants, we need to collect a few important documents and complete some compliance requirements. This checklist covers everything you need to submit. All items are required unless otherwise noted. Phase 01
Personal and Employment Documents
Identity, contracts and payroll setup

Professional Headshot Required
A clear, professional photo to be used for your Staff ID. Please ensure no filters or edits have been applied to the image. Natural lighting and a plain background work best.

Completed Health Declaration Form Required
Complete and upload the Health Declaration Form via Employment Hero. The form will be available in your onboarding tasks once you have logged in.

Signed Employment Contract Required
Your employment contract will be issued to you through Employment Hero. Please review, sign, and return it in full before your start date. Contact if you have any questions before signing.

Bank and Superannuation Details Required
Complete your bank account and superannuation fund details via Employment Hero so we can set up your pay correctly from day one.

Compliance and Screening
Mandatory checks for NDIS and child-related work

NDIS Workers Check (Yellow Card) and Working with Children Check (Blue Card) Required
Both an NDIS Workers Screening Check (Yellow Card) and a Working with Children Check (Blue Card) are required for this role. If you do not currently hold both, you can make a combined application through the Queensland Government.
Apply for one or both checks (including the combined application option) at the Queensland Government Blue Card for disability work page. If you already hold either card, the same page includes instructions to link it with Inspired Living QLD Pty Ltd.

Police Check (National Criminal History Check) Required
A copy of your completed Police Check is required. This must have been completed within the last 12 months. Checks older than 12 months will not be accepted.

Driver's Licence (Front and Back) Required
Please upload a clear image of both the front and back of your current driver's licence via Employment Hero.

Vehicle and Transport
Required for community-based support work

Car Registration Certificate Required
Please provide a current copy of your vehicle's registration certificate.
To obtain your registration certificate, visit the Queensland Transport and Motoring website.

Comprehensive Car Insurance Required
Verification of full comprehensive insurance cover for your vehicle is required. Please upload your current Certificate of Insurance or policy confirmation.

First Aid and CPR Certificate Required
Please provide your current First Aid and CPR certificate. First Aid must have been completed within the last 3 years. CPR must have been completed within the last 12 months. If your certificates are out of date, please renew them before your start date.

Please provide a copy of your Certificate III in Individual Support or a comparable qualification in a relevant field. If you hold a higher qualification, please submit that instead.
